Bar end weight problem.

I have lost one lt. bar end weight during transport. the replacement has kept becoming unscrewed, and now will now tighten. the standoff is a little more that 1/16". I have never worked on one, any ideas on the fix?

The threads must be stripped, are the threads in the ring or in the weight, in the bar?
